​WAIVER AND RELEASE OF LIABILITY

I, the undersigned and identified person, desire to participate in firearms training
and usage with Chaim Stitzer (the “Instructor”). As an essential component of my
consideration to participate in the activities included in and surrounding the
instruction, I hereby enter into and agree to be bound by the terms of this
Agreement for myself, my assigns, heirs, next of kin, or my estate. I HEREBY
ACKNOWLEDGE, UNDERSTAND, AND AGREE TO THE FOLLOWING:

1. I fully understand that I and others will have access to and must handle firearms
and ammunition. I further acknowledge that handling firearms and using firearms
can INVOLVE RISK AND DANGERS of bodily injury, serious bodily injury, and
DEATH. I fully understand these risks and dangers may be caused by my own
actions, or my own inactions, omissions, the actions or inactions of other
participants, and the Instructor, the conditions in which the handling of firearms
takes place (including but not limited to the firing range and its equipment and
other firearms facilities or equipment), and/or the negligence of the Instructor.
Specifically, at risk due to exposure to lead, gunpowder and/or the concussive
effects of discharging firearms are individuals with certain medical conditions such
as heart-related illnesses, hearing and vision issues, hypertension, pregnancy,
nervous disorders, and brain injury. I hereby FULLY ACCEPT AND ASSUME ALL
SUCH RESPONSIBILITY FOR THE RISKS DESCRIBED ABOVE.

2. I Agree and Warrant that I will examine and inspect each activity or piece of
equipment in connection with the activity (including but not limited to firearms,
ammunition, holsters, firing range, firing range equipment, hearing protection, eye
protection, or any other equipment used). I hereby FULLY ACCEPT AND ASSUME
ALL RESPONSIBILITY FOR THE RISKS of using any facility or equipment used,
regardless of the origin or ownership of the equipment. If I observe any activity or
equipment I consider hazardous or potentially harmful to myself or others, I will
notify the Instructor immediately, and I will refuse to take part in any activities I
consider unsafe until the condition has been corrected to my satisfaction.

3. I HEREBY VOLUNTARILY AFFIRM AND AGREE that I FULLY WAIVE, RELEASE,
INDEMNIFY, HOLD HARMLESS, INSTRUCTOR, or any other person or entity
providing a place to engage in firearm training, usage, or activities, and
their or its owners, employees, managers, officers, agents, assigns, and their
contracted entities involved with the Instructor, from any and all liability, claims,
demands, actions, rights of action, cause of action, or legal proceeding whatsoever,
whether personal to me or to a third-person should I become injured from
whatsoever cause (whether physically, financially, or emotionally) or cause injury to
others while I am involved in activities with Instructor, or coming or going to the
activity. I further FULLY WAIVE, RELEASE, INDEMNIFY, HOLD HARMLESS and
DISCHARGE INSTRUCTOR, or any other person or entity providing a place to
engage in the training activities, and their, or, its owners, employees,
managers, officers, agents, assigns, and their contracted entities involved in the
training activity from any and all liability, claims, demands, actions, rights of action,

cause of action, or legal proceeding whatsoever, whether personal to me or to a
third-person based upon any injury that may be claimed, by me or any other
person, to have arisen out of the undersigned participation in the training activity or
anything that the undersigned may have been taught, learned, not taught, or did
not learn from the Instructor.

4. Further, I ACCEPT AND ASSUME ALL RISK AND RESPONSIBILITY from the use of
any borrowed, rented, or loaned firearm(s) I may use. It is my duty to inspect any
such firearm(s) for proper functioning prior to my use of any such firearm(s). I
FULLY WAIVE, RELEASE, INDEMNIFY, HOLD HARMLESS AND DISCHARGE
INSTRUCTOR, or any other person or entity providing a place to engage in
the training activities, and their, or, its owners, employees, managers, officers,
agents, assigns, and their contracted entities involved with the Instructor, from any
and all liability, claims, demands, actions, rights of action, cause of action, or legal
proceeding whatsoever, whether personal to me or to a third-person should I
become injured from whatsoever cause (whether physically, financially, or
emotionally) or cause injury to others while using an owned, borrowed, rented, or
loaned firearm(s) while I am involved in training activities, or coming or going to
any activity with the Instructor.

5. I acknowledge, affirm, and agree that the purpose of the training is solely for
educational and safety purposes, and I am committed to adhering to all state and
federal laws in connection with legal firearm ownership and possession, and the
legal and justified use of force or deadly force.

6. I acknowledge, affirm, and agree that I have had sufficient opportunity to read
this entire WAIVER and RELEASE of my rights, that I understand its content, and
that it is legally binding, and that I executed it freely and voluntarily, and intend
this document to be a complete and unconditional release of all liability to the
greatest extent of the law and agree that if any portion of this Agreement is held
invalid, the remainder of the Agreement shall continue in full force and effect.
